How It Works

Membership Fee: $750/year per patient
Choose the payment option that works best for you:

• Pay in full when you enroll.
• Start with $400 and pay the rest in quarterly installments.
• Set up automatic quarterly payments ($175/quarter).

Please note: Membership fees are not reimbursed by insurance and do not replace health coverage.

Is This Covered by Insurance?

Patient 360 is a membership-based program and not an insurance plan. While CPPC accepts Medicare and many insurance plans for standard services, this membership offers additional amenities not covered by insurance. You’ll still need separate coverage for hospital visits, diagnostics, or specialist care.
