Preparing For Your First Visit

Thank for trusting CPPC with your healthcare! Here are a few things to remember to prepare for your first appointment with us:

  • Please arrive 20 minutes early for your first appointment to check-in and register with us
  • Please bring your photo ID and insurance card (primary and secondary if applicable)
  • If you have an HMO plan, you must first designate a CPPC doctor to be your primary care physician (PCP) through your insurance.
  • This allows us to bill your insurance for services you receive here and refer you to specialists when needed. Co-payments are due at the time of your visit
  • Because your insurance coverage is unique to you, please call the number on the back of your card if you have any questions regarding your coverage and financial responsibility prior to your first appointment
  • Bring a list of any medications and vitamins you are currently taking
  • Bring any medical records from previous health practitioners, if you do not have access to them we can request them on your behalf
  • Prepare a list of any and all concerns you wish to discuss with your provider
  • Your records pertinent to seeing a pulmonary specialist
  • Any scans or imaging you may have related to your pulmonary concern (i.e. chest x-ray, CT scan, PET scan)
  • If you have an HMO, please be sure you have an authorization to see one of our pulmonary specialists
